Lateral suture repair, also known as extracapsular repair, is a common surgical procedure for treating cranial cruciate ligament (CCL) injuries in dogs.
This technique stabilizes the knee joint using a strong suture placed outside the joint to mimic the ligament’s function. It is typically recommended for small to medium-sized dogs and those with less active lifestyles. While recovery requires rest and rehabilitation, lateral suture repair can offer an effective alternative to restore mobility and reduce pain in affected dogs.
Understanding Extracapsular Suture Techniques
Extracapsular techniques use synthetic suture materials of various types (such as nylon, polypropylene, FiberWire®, and Ethibond™) to create a stabilizing structure. This stabilization method attempts to mimic the function of the original CrCL, counteracting the abnormal forces that develop after ligament rupture.
One of the most commonly used extracapsular techniques is Lateral Suture Stabilization (LSS). This method involves placing a single or multiple suture loop (s) outside the joint, typically anchored at the lateral fabella of the femur and passed through a drilled tunnel in the tibial crest. When appropriately tensioned, these sutures help control excessive joint movement.
It is important to note that extracapsular stabilization is not an artificial ligament replacement. The suture serves as a stabilizer and sometimes will gradually loosen or break over time. The long-term joint support tends to rely on thickening of the joint capsule and scar tissue development. Peak recovery is typically seen between 10-12 weeks postoperatively

At VBJS, we almost exclusively recommend TPLO, but extracapsular stabilization may be considered for selected patients based on size, activity level, anatomy, concurrent conditions, and treatment goals.
Recovery from lateral suture repair in dogs typically takes around 10-12 weeks, with gradual improvement over time.
In the first few weeks, strict rest and limited activity are crucial to allow proper healing.
By weeks 6-8, controlled leash walks and gentle rehabilitation exercises help rebuild strength.
By week 12, most dogs regain good mobility, though full recovery may take longer for some. Follow-up exams and a structured rehabilitation plan ensure optimal healing and long-term joint stability.
The cost of lateral suture surgery for dogs varies based on factors such as location, veterinary clinic, and the dog’s size. Expenses typically include pre-surgical exams, anesthesia, the procedure itself, medications, and follow-up care. While generally more affordable than other CCL repair surgeries, costs can still vary significantly.
VBJS would generally always recommend TPLO.
The success rate of lateral suture surgery for dogs with cranial cruciate ligament (CCL) injuries is generally 85-90% for small to medium-sized dogs.
Most dogs regain good mobility and experience pain relief, especially with proper post-operative care and rehabilitation. However, larger or highly active dogs may have a higher risk of complications, such as implant failure or arthritis development.
Following veterinary guidelines for recovery and physical therapy greatly improves long-term outcomes.
Potential Complications of Extracapsular suture
While this surgery can be effective with careful patient selection, improper execution can lead to complications, including:
Around 10 percent of cases require further surgical intervention.
